The Philippine Statistics Authority (PSA) announces the release of the Questionnaires used in the conduct of the Community-Based Monitoring System (CBMS) in selected Local Government Units for the years 2021 to 2023 on its official website, accessible to the public starting today. These questionnaires include the Barangay Profile Questionnaire, Household Profile Questionnaire, and the Geotagging of Service Institutions and Infrastructure, Government Projects and Natural Resources Questionnaire.

This initiative is in line with the agency’s commitment to transparency, accessibility, and engaging the public in meaningful data collection in nation-building. Intended to gather vital information encompassing a wide range of socio-economic factors, the 2021 to 2023 CBMS Questionnaires gather information from barangays and households covering various sectoral themes including health, education, employment, housing, and other thematic areas.
